Form Instructions:

This form is used by flight instructors at Leighnor Aircraft LLC to evaluate pilots for rental checkouts in all rental aircraft. A satisfactory result is required on all knowledge topics or flight maneuvers listed below.

Based on checkout location, if this checkout is a first-time pilot with Leighnor Aircraft, the instructor needs to go through the following information with the new pilot so the pilot understands how to do things at Leighnor Aircraft

Before Flight Operational Items to Cover:

Leighnor Aircraft Rental Agreement has been completed and submitted online
If the pilot is under the age of 18, a parental consent form has been completed and submitted online
For Prescott, the pilot has completed and submitted a Gate Code Request form online
The pilot has money on account, a credit card on file, or payment arrangements made to pay after the flight
After hours access information for the location and aircraft keys
For Cottonwood, access information for the terminal and office
Where the airplane is parked and how to access the airplane
Preflight and fueling of hangered aircraft
How to know what type of oil to use for the airplane
Familiarization with the airplane boards
Definition and submission of airworthiness squawks
How to schedule an airplane with Flight Schedule Pro
How to submit fuel reimbursements
Awareness that the Leighnor Aircraft Rental Application form must be completed and submitted online before solo flight rentals allowed
Parking and securing aircraft

The applicant failed to demonstrate satisfactory knowledge of one or more procedures and flight maneuvers and further training is required.
